Cylindrical Photo Help

I was wondering if anyone can tell me how to make a Cylindrical Photo like the ones I have listed at: www.360ipt.com/logohelp.htm .

I would like to use it on my business cards, because the logo I currently have "looks like I offer a slideshow" blush

I like the angle of the 1st photo.

I think the 2nd photo on the page was taken from a PanoGuide user's website. I'm not trying to use it, so please don't get mad! I just wanted to use it as an example, because trying to EXPLAIN what I wanted wasn't getting me anywhere. I can't get a graphics designer to touch the project, and this is my 4th month trying to create it on my own with no success.

I thought flexify might do it:

It has just about very other transformation, but no cylinder rings sad

Anyone else know how this is done?
